Billing Notice December 2023

Dear Unified Family Therapy Clients,

Warm greetings as we approach the new year! We hope this message finds you in good spirits.

At Unified Family Therapy, we are dedicated to providing you with exceptional care while also striving to keep our costs as reasonable as possible even with the rise of inflation. To ensure we maintain the quality of our services and remain sustainable, we’re making a few minor adjustments to our billing procedures:

Implementation of Card Usage Fee: Starting 1/1/2024, a 3% fee will be applied to all transactions made using cards. To help minimize this fee, we kindly encourage payments to be made in cash or check.

Fee Adjustments: Please note that our session fee will be increasing from 150 to 155. We’ve taken great care in evaluating these changes to balance our commitment to you with the necessity to cover our operational expenses effectively.

Annual Paperwork Updates: To streamline our administrative processes and ensure accurate records, we kindly request all clients to update their paperwork annually. Your cooperation in this matter will significantly contribute to the efficiency of our services. Please plan to arrive 10-15 minutes early to your appointments at the beginning of the year to update your paperwork. Alternatively, we can send you a copy electronically that you can sign and return at your convenience.

Insurance Information Update: Your insurance details play a crucial role in facilitating a smooth billing process. We kindly ask you to update this information and remain aware of any changes in deductibles that might have an impact on your coverage.

Out-of-Network Coverage: For services covered out-of-network, self-payment will be required. Upon your request, we will provide a detailed superbill for submission to your insurance provider. Insurance will reimburse you directly.

Court-Affiliated Therapy: Please note that we do not intervene in court orders regarding payments or responsibilities.

We sincerely appreciate your understanding and cooperation as we make these necessary adjustments. If you have any questions or need further clarification, our dedicated billing department is available at 801-960-7910 or via email at [email protected]

Thank you for being a valued part of the Unified Family Therapy community. Your trust and support mean a great deal to us as we continue striving to offer you the best care possible.
